ADMISSIONS PROCEDURES  (Checklist) 

An application will not be considered until the school receives all of the items below: 
[] an Application Form 
[] a Student Information Form (for grades 6-12) 
[] a Code of Conduct Form (for grades 3-12) 
[] a Medical Information Form for each student 
[] Proof of Canadian citizenship or landed immigrant status for each student 
[] Proof of age for each student (copy of birth certificate or passport) 
[] a copy of the most recent report card for each student 
 

 
Once accepted, we require:
 

a)      A completed Pre-Authorized Debit form (usually with a voided cheque attached) or a lump sum payment for the whole school year.

b)      A registration/volunteer deposit of $300. This deposit is refunded only when 30 volunteer hours have been completed. If the student withdraws from the school, the deposit is not refunded.
